Train 408 departed Seattle with 450 passengers, many bound for the Rose Bowl in Los Angeles. The train transferred to the northbound track at Sumner to get around a freight train derailment at Puyallup. At Meeker (edge of Puyallup) the train was to transfer to another track (Third Main) and pass around the derailment.

When 408 reached Meeker the engineer had forgotten about the orders to go to the Third Main and hit the 15 mph switch at 49 mph. Following the accident the engineer retired on the spot. Nine cars derailed, eight remained on the rails. View shows several of the derailed cars.

A substitute train made up in Seattle picked up the 8 cars in Puyallup that did not derail and continued south 5 hours late. Southern Pacific held the Cascade at Portland to connect.
